Hogwarts has had the tradition of appointing a Head Boy, a Head Girl, and one or two Prefects for each house. These are student positions of power and are to be taken seriously.
Therefore, there will be some requirements before you can submit your application:
You must have an approved character bio and be sorted into a house. PREFECT: Your character must be a fifth year with at least 50 decent posts before your application will be considered. HEAD: Your character must be a seventh year with a solid post count before your application will be considered. - Decent posts are defined as any that are of your character roleplaying. You must be an upstanding member with a clean record. This means no current warnings and/or bans. You must be enrolled in classes at the time of application.

Your application must be turned in before the start of the new term, so applications are taken all term long until announcement at the next Start of Term feast.
These positions are Staff elected as no student elections were ever evidenced in the books.
If you are already holding a position such as Quidditch Captain, etc., you will be considered at the bottom of the list. This is to give others a chance to help the student body. Regular Quidditch players may still be highly considered. Please do not take this to mean that you shouldn't apply!
A Head Boy or Girl is a representative of the entire student body. They know no house and are to make unbiased decisions. They are usually chosen from experienced students of years four and up.
A Head Boy/Girl resides over the Student Clubs area as well as their Common Room, and is allowed to award or deduct a minimal amount of points for reasonable or unreasonable behavior. [See House Point Bible for details.] They are also to report any suspicious activity outside of the Student Clubs to an administrator.
Abuse of this power will result in position stripping and disciplinary action.

A Prefect is a representative of their house. They are expected to set a good example for their house, and thus should try and be unbiased.
Prefects reside over their Common Rooms. If different gendered Prefects, their respective Dorms as well. If not gendered and only an individual, they reside over all parts of the house as a moderator. A Prefect is allowed to award or deduct a minimal amount of points for reasonable or unreasonable behavior. [See House Point Bible for details.] They are also to report any suspicious activity within or outside of their Common Rooms to their HoH or administrator depending on the location.
